Una de las cuestiones analizadas en el estudio, realizado a lo largo de dos meses, es la evolución de los anticuerpos, que era uno de los objetivos. Se ha observado una tasa global de seroconversión (personas que no tenían anticuerpos en la primera oleada y que los han desarrollado a lo largo del estudio al tener contacto con el virus) del 0,9% entre la ronda 1 y la ronda 2, un porcentaje algo menor (0,7%) entre la ronda 2 y la ronda 3. Estas cifras reflejan la baja aparición de nuevas infecciones tras el confinamiento.
Por otra parte, también se ha observado seronegativización, es decir, ausencia de anticuerpos IgG detectables en personas que los tenían previamente, siendo globalmente de un 7,1% entre la ronda 1 y la ronda 2, y de aproximadamente un 14% al analizar todo el periodo del estudio, aunque este último dato requiere confirmación. La "pérdida" de los anticuerpos fue más frecuente en personas que no habían tenido ningún síntoma (11,0% entre las rondas 1 y 2, con información más precisa) y mucho menos frecuente en los participantes con una PCR positiva (0,5%) y en aquellos que describieron pérdida súbita del olfato o del gusto (2,6%).

El mapa de participantes con síntomas COVID-19 (personas con tres o más síntomas o con pérdida súbita del olfato) en las dos últimas semanas del estudio da una idea de la evolución más reciente de la epidemia. Mientras el porcentaje de sintomáticos ha disminuido sustancialmente entre la primera y la segunda ronda, se ha observado un leve incremento en la última ronda, lo cual podría ser fruto de la mayor movilidad de la población tras el periodo de confinamiento. Los autores del artículo de ‘The Lancet’ consideran que los resultados obtenidos en España, un país con una onda epidémica intensa en el que las cifras de seroprevalencia son bajas, reflejan la dificultad de obtener una inmunidad de rebaño a corto plazo. En este sentido, el comentario que acompaña al artículo de "The Lancet" a cargo de investigadores de centros de referencia en Ginebra (Drs Isabella Eckerle & Dr. Benjamin Meyer) señala que "sería poco ético someter a la población y al sistema sanitario a una mayor presión para conseguir esta inmunidad de grupo".

Variabilidad geográfica
Según se señala en dicha publicación, y como ya se trasladó en la difusión de los datos preliminares de las primeras oleadas, existe bastante variabilidad geográfica en el porcentaje de seroprevalencia. En la presentación de resultados finales del estudio se ha confirmado que entre el 74% y el 89% de los participantes que referían haber tenido una PCR positiva al menos dos semanas antes del estudio se detectan anticuerpos IgG contra el coronavirus. Entre las personas que refieren haber presentado síntomas compatibles con la enfermedad, la seroprevalencia aumenta con el número de síntomas y es particularmente alta en las personas que refieren pérdida súbita del olfato y/o del gusto (40%-41%). Finalmente, se confirma que en torno a un 2,5% y un 2,8% de los participantes que no refieren ningún síntoma presentaron anticuerpos IgG, lo que refuerza la existencia de infecciones asintomáticas.
Los participantes que comunicaron haber tenido contacto con un caso COVID-19 confirmado, o con una persona con síntomas compatibles, presentaron una prevalencia de anticuerpos superior a la población general. Destaca la prevalencia en personas que han convivido con un paciente COVID-19 confirmado que se sitúa entre el 27 y el 31% en las distintas rondas. Los convivientes con personas con síntomas compatibles con la enfermedad también presentaron una seroprevalencia más alta que la media (15%). En personas que refieren haber tenido contacto con un caso confirmado fuera del hogar, la seroprevalencia oscila entre el 10% y el 15% en las distintas rondas.

Las tres rondas del estudio, impulsado por el Ministerio de Sanidad y el Instituto de Salud Carlos III, dependiente del Ministerio de Ciencia e Innovación, y en el que han colaborado los sistemas de salud de todas las comunidades autónomas y las ciudades de Ceuta y Melilla se han desarrollado entre el 27 de abril y el 11 de mayo; del 18 de mayo al 1 de junio, y del 8 al 22 de junio.
Ha incluido un total de 68.296 participantes, de los cuales 54.858 han participado en las tres rondas, lo que representa una adherencia de casi el 90%. Además, el 91% de los participantes ha proporcionado al menos una muestra de sangre para el análisis.
